HH-43B 60-0268 crashed during a scramble when the crew chief failed to remove the MD-3 power unit cable and that began the crash sequence when they lifted off. The crew chief was hit by a piece of blade during the crash but was not badly injured. This is the same helicopter that was named "South Sea Sue" during operations with Photo Mapping in New Guinea prior to being transferred to Clark AB, PI in 1964.
